Purpose
The website brings together practical and current tools, strategies, and information designed to help physicians and other health care providers in Washington State care for children, youth and families with special health care needs.
The website is also designed to support youth and families in understanding how to partner with their health care providers, understand their health conditions, and make informed health care decisions.
The Washington State Medical Home Website partners with the Washington
State Department of Health Children with Special Health Care Needs Program,
American Academy of Pediatrics National Center of Medical Home Initiatives
for Children with Special Needs and other
states to assure every child and youth with special needs has a
medical home.

Content
New information and resources are being added to the Medical Home website on a continual basis. Feedback from users is important in prioritization of updates and additions. Suggestions for topics and materials come from physicians, nurses, families, family resources coordinators, state agencies, university faculty, community Medical Home Leadership Network teams and others. MHLN staff work with state experts in each content area to determine which resources to include.

Personnel and Funding
The Medical Home Website is managed and maintained by staff at the Medical Home Leadership Network, Center on Human Development and Disability(CHDD), University of Washington, in cooperation with CHDD's Leadership Education in Neurodevelopmental Disabilities (LEND) program.
Primary funding comes from the Washington State Department of Health, Children with Special Health Care Needs (CSHCN) Program which actively collaborates on the website through CSHCN Program staff and contractors. Funding from the US Maternal and Child Health Bureau (Grant HO2MC 00079) and the American Academy of Pediatrics National Center of Medical Home Initiatives for Children with Special Needs supported the development and expansion of the website.
