Guidelines

  • Bright Futures:  Bright Futures: Guidelines for Health Supervision of Infants, Children, and Adolescents (3rd Edition. 2007 ) (Prior versions 1994; 2000; 2002)

    • Funded by Maternal Child Health Bureau, the publication provides comprehensive health supervision guidelines, including recommendations on immunizations, routine health screenings, and anticipatory guidance. Available in print and on the Web.

Washington State Health and Development Supervision for the Well-Child Visit


When a primary health care provider uses a specific health and development supervision form during a well-child visit, it results in a more comprehensive and complete health visit.

Washington State well-child exam forms for primary care providers include a detachable second page parental handout with anticipatory guidance and emerging developmental milestones for each well child visit age.  The forms meet documentation guidelines for the Healthy Kids Early and Periodic Screening Diagnosis and Treatment (EPSDT) program.

CHILDHOOD IMMUNIZATIONS

Washington State Immunization Registry   www.childprofile.wa.gov


A web application that allows enrolled primary care providers to conveniently search for patients in the CHILD Profile statewide immunization registry and to view patient immunization records. Authorized users can:

  • Add and edit patient records
  • Review vaccination records, adding or editing current and historical immunizations
  • Maintain facility-specific records on vaccinators, physicians, and lot numbers
  • Record required information for each VIS given
  • Access the tools to support a reminder/recall system

Anticipatory Guidance for Parents - Health Messages to Offer at Well-Child Visits

Help parents become active partners in their children's health by translating the health supervision visit into everyday language. Families learn what to expect at each well-child visit and how to prepare for the next visit, the types of questions and concerns to talk over with the health professional, and ways to prepare for their child's next stage of development.

Easy-to-read sheets designed to help families promote the health and well-being of their children.  Information on topics such as social development, child care, safety, eating and physical activity in 4 age groupings: infancy, early childhood, middle childhood and adolescence.

 

Washington State

    Available in English and Spanish.

    CHILD Profile sends health promotion materials to all parents of children birth to age six born in Washington State.  There are 17 mailings, timed to correspond with the AAP-recommended schedule of well-child visits. Children not receiving mailings can be enrolled. Click on the following link to sign up:

    The materials contain age-specific information about growth, development, safety, nutrition, and other parenting issues.

    Each mailing contains an age-appropriate letter and may contain other useful brochures and pamphlets (view individual letters and materials).
