Leadership Network

Support for MHLN Community Teams

The purpose of this section is to support the activities of the county-based, interdisciplinary Medical Home Leadership Network (MHLN) teams in Washington State.  Other groups working to improve family-centered, coordinated care for children and youth with special health care needs may also find the information in this section useful. 

MHLN teams identify and carry out at least one strategy to strengthen medical homes for children with special health care needs in their community.  Activities often include outreach to other health care and service providers to increase awareness of community resources as well as other strategies that support medical homes.  Many teams, as a team or in their other roles, write grants for additional funding to support local activities.

Brief newsletters on tools and resources for primary care office practice, screening and anticipatory care guidance, early identification and diagnosis of special health conditions. Counties -- often the local health department-- can individualize the Notes with contact information for local support services such as the public health CSHCN Coordinator.

Outreach Tips

Tips on successful strategies to reach local provider and other audiences.

Presentations

State and national presentations on medical homes, including screening and identification of children with special health care needs. Also information about medical home videos.

Team Logistics

Template for MHLN team workplans and the paperwork to receive the annual team honorarium.  Prior to each conference, logistical information about the event is posted.
