Purpose
This page provides a list of links to data sources that can be used by the Medical Home Leadership Network (MHLN) teams and others to use in program planning, writing grants, and tailoring presentations to community and state groups.
What You'll Find
Data sources that provide valuable Washington State or County population characteristics, service use and/or needs of one or more of the following groups:
- children and youth with special health care needs and their families
- children and youth in general
- and people with disabilities and special health care needs.
In addition, some links to national resources have been included to provide helpful context for developing grants related to medical homes for children and youth with special needs.

The Health and Wellbeing of Children: A Portrait of States and the Nation 2005
Overview of broad health characteristics for
children and brings together national and state-level data for the first
time. The chartbook, produced by
the Maternal and Child Health Bureau, is based on data from the
National Survey of Children's Health and focuses on measures of
children's physical and mental health, health care, and general
well-being as it relates to their families, schools, and neighborhoods.
Information for each state, easy-to-follow bar graphs, and breakdown
analyses by race and ethnicity are included.
