- Presentations to Eastside primary care clinics about community resources available to physicians and families who have children with special needs
- Part of team meeting with Medicaid Healthy Options case managers
- Team physician, her clinic nurse and clerk from the Eastgate public health clinic participating in the 2006-07 Children's Health Improvement Collaborative to prevent and treat childhood obesity. Adapting materials for families in Spanish.
- Team physician and CSHCN Coordinator participating in regular planning meetings held by the Adolescent Health Transition Project to explore how to help transition patients to adult care.
- Exploring whether to begin to send out Child Health Notes to a subset of primary care providers in King County.
- Providing education to county public health nurse team and supervisors on medical home concept.
- Team physician involved in a redesign of her clinic's care teams
- King County and Pierce County CSHCN Coordinators helped Dept of Health develop decision tree diagram to help primary care providers know where to refer a child with health or developmental concerns for help accessing services. Diagram will be developed into a Child Health Note, spring 2007.
- Working on developing medical home teams in other county health department sites.
- June 2006 presentation by MHLN staff organized by the team for a group of 20 Eastside clinic nurses, school nurses, and family resources coordinators on resources available through the WA Medical Home Website and the Adolescent Health Transition Project.

Current and Upcoming Activities
- Pierce County and King County CSHCN Coordinators helped Dept of Health develop a decision tree diagram to help primary care providers know where to refer a child with health or developmental concerns for help accessing services. Diagram will be developed into a Child Health Note, spring 2007.
- Team FRC helped plan spring 2007 MHLN Conference.
- Medical Home Tool kits modeled after Children’s model
created. Presented kits and did short medical home presentations at 10 Pierce
County provider sites. Presenting team were Mary Bridge RN,
CSHCN Coordinator, and FRC
- Single Point of Entry for Family Resources Coordination
has been very helpful in our community. Referral numbers are up.
- Community is expanding outreach efforts for screening children
for developmental delay through Childreach.
- A brief review regarding how the facilitation of "Medical
Home" concepts are provided for in a teritary setting was completed
at Mary Bridge Children's Hospital and Health Center. Mary Bridge
Children's Hospital and Health Center provides comprehensive pediatric
health services for children residing in Southwest Washington. Many
components of facilitating the Medical Home concept were found to
be present and working well at Mary Bridge which organizationally
considers family centered care and partnership with families and
professionals as a primary foundation of care.
Past Activities
- Creation of Pierce County Resource List and CSHCN Pierce County Referral List.
- Sent out Child Health Note on Overweight in Mary Bridge Newsletter "KiteStrings" Mailed to SW WA pediatric primary care providers.
- Article placed in Multicare’s “Med News” which is widely circulated to Pierce County Health Care Providers and into the community. This article described “Medical Home” and offered contact numbers for more information.
