The NCMHI has also developed a "Building Your Medical Home" toolkit to support development and/or improvement of a pediatric medical home. It also prepares practices and clinicians to apply for and potentially meet the National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) recognition program requirements. The AAP created a crosswalk between each of the toolkit building blocks and the NCQA 2008 PPC®-PCMH™ and PCMH 2011 recognition program 'must pass' elements.
