Medical Home Measures
Medical Home Recognition and Accreditation Programs
Several national organizations have developed or in the process of developing programs
that recognize and/or accredit health care organizations as medical
homes according to specified sets of standards.
There is not yet an "official" certification in Washington State to enable providers and health care organizations to receive extra reimbursement for "medical homeness." However, many providers and clinics are interested in and working toward medical home recognition. You can see which providers or clinics in WA have achieved recognition through the National Committee for Quality Assurance's (NCQA) patient centered medical home program at:

The National Center for Medical Home Implementation website has information on the most widely recognized medical home recognition and accreditation programs.
