SEAS Care Coordinator (1) provides phone screening, (2) initiates shared
intake process including summary of needs, previous evaluations, current
services, and consent to exchange information, and (3) assures:

 

  • Referral to Early Support for Infants and Toddler (ESIT) for children
    under 3 with developmental concerns
  • Referral to school district (Focus of Concern) for children 3 and over
    with developmental concerns
  • Referral to Children with Special Health Care Needs (CSHCN) services
    and/or appropriate community resources/agencies for children with
    additional special health care needs
  • Follow-up with family and primary care provider regarding referral
    process and outcomes, offering additional assistance if needed
